Severe hypoxaemia, characteristic of COPD with severe pulmonary hypertension, is due to a combination of greater ventilation–perfusion mismatch, increased intrapulmonary shunt and reduced venous oxygen tension, with negligible hypoxic pulmonary vasoconstriction regulation.
